We are currently in-network providers for Medicare, Blue Cross, Blue Shield, and United Healthcare. Please note that many other insurance plans have out-of-network benefits, and we will be happy to work with you to find an affordable solution to your therapy needs. Effective August 1, 2010, we no longer accept worker’s compensation insurance.
For other third party claims, including auto injuries, payment is due at the time of your visit (no exceptions).
In the state of California, a medical diagnosis is required for physical therapy treatment beyond the initial evaluation. Also, most insurance companies require a physician referral to cover physical therapy services. The referral may be written on any MD or PT referral form, as long as it includes a diagnosis and MD signature.
